The Advantage Program is a Medicaid state waiver program that allows individuals living in Oklahoma who are nursing home level of care to remain living in their home with assistance. Advantage services include case management, personal care assistance and nursing. The case manager monitors health, fall risk, education needs, safety needs such as needed equipment, increase or decrease in services as needed, assists with the coordination of care with other providers, communication between agencies, monthly monitors and annual evaluation/reassessment to name a few. The RN performs a nursing assessment every six months. The Personal Care Assistant can assist with housekeeping, laundry, shopping/errands, personal care and meal prep as needed. Other services can include home delivered meals, equipment, nutritional supplements, emergency response equipment and more if qualified. Other programs include CD-PASS and State Plan Personal Care.
